Booking Request Terms

Submitting this form does not reserve my event date. My booking request is subject to final confirmation by Wear Integrity Entertainment. A nonrefundable retainer is required to reserve my date, and the remaining balance is due 72 hours before the event.

I agree to provide adequate space and safe access for delivery and setup. Power and Wi-Fi are required only when specified for my selected services.

​A final service agreement with full terms will be provided for my review and signature before my booking is confirmed.
